LTC Pharmacist – Syracuse, NY
Salary: $135,000 - $145,000
Schedule: M – F 9 AM – 5:30 PM (Rotating Saturdays 9 AM – 13:00)
Reports to: Pharmacy Manager
Type of Role: Direct Hire
We are currently looking for a Pharmacist to join an LTC Pharmacy in Syracuse, NY in a full-time position. You will be joining a great team within a fantastic working environment.
The Pharmacist is responsible for providing pharmaceutical care to residents of a long-term care facility. This includes dispensing medications, counseling residents and their families on medication use, and managing medication profiles. The Pharmacist must also work closely with other healthcare professionals to ensure that residents receive the best possible care.
Responsibilities:
- Dispense medications to residents in accordance with physician orders and pharmacy policies and procedures.
- Counsel residents and their families on medication use, including potential side effects, drug interactions, and proper storage and disposal of medications.
- Manage medication profiles for residents, including monitoring for drug interactions, allergies, and adverse drug events.
- Work with other healthcare professionals, such as nurses and physicians, to ensure that residents receive the best possible care.
- Maintain up-to-date knowledge of medications and pharmacy regulations.
- Participate in continuing education programs to maintain and improve skills.

Requirements:
- Bachelor’s degree or higher from an accredited college or university in Pharmacy
- Current New York Pharmacist License
- Experience working in a long-term care pharmacy is preferred but not essential
