Do you want to use your clinical expertise to improve patient care within a collaborative primary care network.
We are seeking a passionate and motivated Clinical Pharmacist to join our partners team in York. This is an exciting opportunity for a pharmacist to play a key role in optimising medication use and improving health outcomes for patients across their PCN.

About the role -
- Work collaboratively with GPs, nurses, and other healthcare professionals to deliver high-quality patient care.
- Conduct comprehensive medication reviews to ensure the safety and effectiveness of treatment plans.
- Manage acute medication requests and ensure seamless medication transitions following hospital discharge.
- Provide expert advice and support to patients on a range of conditions.
- Contribute to achieving PCN objectives and Quality Outcomes Framework targets.
- Participate in the development and delivery of medication management clinics and educational programs.
We are looking for someone who -
- UK Registered Pharmacist
- Has experience working in a primary care setting or a strong desire to transition from secondary care.
- Possesses excellent clinical knowledge and a commitment to patient safety.
- Has strong communication, collaboration, and interpersonal skills.
- Is a team player with a proactive and positive attitude.
